Current Status of Metal Stents for Malignant Gastro-Duodenal Obstruction

چکیده
Metal stent insertion to relieve malignant gastroduodenal obstruction is an effective alternative to surgery, but there are some problems associated with existing metal stents for malignant gastroduodenal obstruction. Common complications of covered and uncovered stents are migration and obstruction, respectively. To date, there are several practice-improved stents, such as partially covered, double-layer, three-layer, and individualized stents designed according to the shape and size of the Gastric Outlet Obstruction (GOO). Choosing the most appropriate type of stent is arduous and should be done mainly in relation to the morphological aspects of the stricture. In general, the stent is implanted by a Through-The-Endoscope (TTS) method, but certain types of stent can only be implanted by a peroral route. After adequate training, endoscopic gastroduodenal stent insertion is a reproducible, simple, safe, and costeffective procedure, with high technical and clinical success rates. *Corresponding author: Ding Shi, Department of Gastroenterology, The First People’s Hospital of Yuhang District, Hangzhou 311100, Zhejiang Province, China, Tel: 0571-4409213; Fax: 0571-86224254; E-mail: [email protected] Received July 16, 2013; Accepted September 26, 2013; Published September 28, 2013 Citation: Shi D, Qiu Xm, Feng Ty (2013) Current Status of Metal Stents for Malignant Gastro-Duodenal Obstruction. Duodenal stents for malignant duodenal strictures.
Duodenal obstruction may be caused by inoperable malignant disease. Symptoms of nausea and vomiting have been traditionally palliated by surgery. The aim of the study was to determine the efficacy of the endoscopic placement of metal self expanding duodenal stents for the palliation of malignant duodenal obstruction. Four patients with malignant gastric outlet obstruction are described. One pat...

Patients with pancreatic cancer frequently suffer from both biliary and duodenal obstruction. For such patients, both biliary and duodenal self-expandable metal stent placement is necessary to palliate their symptoms, but it was difficult to cross two metal stents. Recently, endoscopic ultrasonography-guided choledochoduodenostomy (EUS-CDS) was reported to be effective for patients with an inac...
متن کاملA unique complication of self-expandable metal stent placement in malignant duodenal obstruction
Intestinal obstruction is a common complication in patients with advanced gastrointestinal malignancies. In the last two decades, endoscopic placement of duodenal stents has become a mainstay of palliative treatment in patients with unresectable obstructive duodenal pathology. Self-expandable metal stents have been reported to have excellent success rates, besides dramatically improve the patie...

An 83-year-old man was admitted to our department, presenting with jaundice, fever, and nausea. CT revealed a pancreatic head tumor with duodenal invasion. Endoscopic transpapillary biliary drainage was unsuccessful due to stenosis at the second portion of the duodenum and tumor invasion to the papilla of Vater. Using a convex linear array echoendoscope, a fully-covered metal stent was placed a...
متن کاملTreatment of malignant gastric outlet obstruction with stents: An evaluation of the reported variables for clinical outcome
BACKGROUND Malignant gastric outlet obstruction (GOO) is commonly seen in patients with advanced gastric-, pancreatic-, duodenal, hepatobiliary or metastatic malignancies. Ten to 25% of patients with pancreatic cancer will develop duodenal obstruction during the course of the disease. Duodenal stenting with self-expandable metal stents is an alternative treatment to surgical bypass procedures. ...
